Enter information about yourself on this card.  Information about the
items you are ordering goes on the previous card.  You may also customize
the text of the ordering letter here and save the information to use later.

• Put your name, address, and daytime phone number in the upper fields
   of the card.
• Use the arrow pointing to the disk icon to save your name, address and
   phone on the disk of your choice.
• Use the arrow pointing from the disk icon to reload your name, address
   and phone information, if you have previously saved it.
• Edit the text of the ordering letter in the field on the bottom half of
   this card.
• Use the Print button to print the order form.
• To go back to the catalog, press the Return button in the lower right
   corner of this card.  Or click the "Whole Earth" icon in the upper right.
